BackgroundObtaining informed consent for participation in genomic research in low-income settings presents specific ethical issues requiring attention. These include the challenges that arise when providing information about unfamiliar and technical research methods, the implications of complicated infrastructure and data sharing requirements, and the potential consequences of future research with samples and data. This study investigated researchers’ and participants’ parents’ experiences of a consent process and understandings of a genome-wide association study of malaria involving children aged five and under in Mali. (...) It aimed to inform best practices in recruiting participants into genomic research.MethodsA qualitative rapid ethical assessment was undertaken. Fifty-five semi-structured interviews were conducted with the parents of research participants. An additional nine semi-structured interviews were conducted with senior research scientists, research assistants and with a member of an ethics committee. A focus group with five parents of research participants and direct observations of four consent processes were also conducted. French and translated English transcripts were descriptively and thematically coded using OpenCode software.ResultsParticipants’ parents in the MalariaGEN study had differing understandings of the causes of malaria, the rationale for collecting blood samples, the purposes of the study and the kinds of information the study would generate. Genomic aspects of the research, including the gene/environment interaction underlying susceptibility or resistance to severe malaria, proved particularly challenging to explain and understand.ConclusionsThis study identifies a number of areas to be addressed in the design of consent processes for genomic research, some of which require careful ethical analysis. These include determining how much information should be provided about differing aspects of the research and how best to promote understandings of genomic research. We conclude that it is important to build capacity in the design and conduct of effective and appropriate consent processes for genomic research in low and middle-income settings. Additionally, consideration should be given to the role of review committees and community consultation activities in protecting the interests of participants in genomic research. (shrink)

Background: Recently, a singular survey titled “Measure of Moral Distress—Healthcare Professionals,” which addresses shortcomings of previous instruments, has been validated. Aim: To determine how moral distress affects nurses and physicians differently across the various wards of a community hospital. Participant and research context: We distributed a self-administered, validated survey titled “Measure of Moral Distress—Healthcare Professionals” to all nurses and physicians in the medical/surgical ward, telemetry ward, intensive care units, and emergency rooms of a community hospital. Findings: A total of 101 (...) surveys were included in the study. The mean Measure of Moral Distress—Healthcare Professionals score for all respondents was 143.0 (standard deviation = 79.8). The mean Measure of Moral Distress—Healthcare Professionals score was 1.75 greater for nurses than for physicians (92.5 vs 161.5, p <.001), and nurses were 2.52 times more likely to consider leaving their position due to moral distress (68% vs 27%). The mean Measure of Moral Distress—Healthcare Professionals score for moral distress was least prevalent in the medical/surgical ward (92.5, SD = 38.2) and highest in the telemetry ward (197.7, SD = 83.6). The intensive care unit ward had a mean Measure of Moral Distress—Healthcare Professionals score mildly greater than the emergency room. Ethical considerations: No participant identifying information or information connecting a survey response to an individual was collected. This study was approved by the Raritan Bay Medical Center’s Institutional Review Board. Discussion: This study provides insight into the level of moral distress in the community hospital setting. Telemetry nurses experience significantly more than nurses in other wards. Telemetry nurses typically manage patients sicker than medical/surgical wards, however do not have the resources of the critical care units. This scenario presents challenges for telemetry nurses and may explain their elevated moral distress. Conclusion: In community hospitals, telemetry nurses experience a considerably greater amount of moral distress compared to their colleagues in other wards. As measured by the Measure of Moral Distress—Healthcare Professionals questionnaire, moral distress continues to be higher among nurses compared to physicians. (shrink)

Cloth, $295.00.The term ‘British Idealism’ underdetermines the interests and geographies of philosophers classed under its heading. It may imply a common goal or, indeed, location. This is misleading. The Biographical Encyclopedia of British Idealism goes a long way in demonstrating the challenge of grouping together philosophers with (...) divergent interests and historical pedigrees, sharing some affiliation with Idealist thinking in Britain from the late nineteenth and early twentieth centuries. Many transcend so-called Analytic and Continental categories, and if not transcend, certainly sidestep them. William Sweet’s volume is at once an archaeological guide to British Idealism and a guide to its relevance today. The volume excavates deeply hidden contributions to philosophy and may destabilize our customary assumptions about the geographical limits of Western philosophy; the contributors to the volume ask readers to extend their community of interest to scholars from Southern Africa, India, and East Asia. Calling philosophers working in India and East Asia “British” is also an important reminder; it has its obvious links to British imperium while at the same time speaking to the influence of Idealist thinking outside the frame of colonial England to thinkers working outside the canonical disputes of Western philosophy.This massive volume, uncompromisingly detailed and comprehensive, does have the elements of a survey, but this is not its intent, nor does one get this impression on reading its entries. This Encyclopedia aims to historically and philosophically contextualize Idealist thought. The first six chapters do just this. They are “Idealism in Britain” (William Sweet); “Idealism in Australia and New Zealand” (Mark Weblin); “The Impact of Idealism in North America” (Leslie Armour); “British Idealism in Southern Africa” (William Sweet); “British Idealism in India” (William Sweet); and “British Idealism in East Asia” (William Sweet). [End Page 146] The remainder of the volume includes entries on well-known Idealist philosophers, lesser known thinkers, and important critics of Idealist philosophy, including the familiar and highly influential Russell and Moore. I will say a little bit about the excellent chapter on British Idealism in India.While readers may have familiarity with Idealism in North America and Britain, it may come as a surprise to learn that Indian philosophers, influenced by a great deal of British philosophy, were particularly keen on Idealism in the late nineteenth and early twentieth centuries. Sweet provides readers with a short history of Indian higher education and the place, for instance, of Kant and Hegel in the philosophy curriculum throughout India in the late nineteenth century; it was not long after that British Idealists were being taught to students in Bombay (Mumbai), Madras, and Calcutta (still important centres of higher education in India) along with Allahabad, Mysore, Pondicherry, and Amalner. Idealists in India, much like their counterparts in Britain, labored beyond academic philosophy, extending their discussions to questions of nationalism in India and contributing to ideas in social and political thought.The attraction of British Idealism (indeed, any “Western” Idealist philosophy) to Indian intellectuals may seem mysterious to some readers. Sweet maintains that Indian philosophy was and is by its own historical sense idealist and so the British variant was not so much as “imported” as finding a natural home. Sweet thinks that the revival of interest in the tradition of Advaita Vedanta can be reasonably traced to the influence of British Idealism’s place in Indian philosophy. There is little dispute that Kant, Hegel, and the British thinkers of the late nineteenth and early twentieth centuries influenced how Indian philosophers understood the discipline’s Western identity and aims. Indian philosophers had begun to enter discussions belonging to another history. Hiralal Halder’s Hegelianism and Human Personality, published in 1910, and Ras-Vihari Das’s doctoral dissertation of 1930, The Self and the Ideal, could easily be taken as titles of works in Western European or North American libraries. Philosophy no doubt includes philosophers who come from many histories, and the discipline may be resistant to simple geographical reduction.Sweet cautions readers that Indian philosophers contributed to the development of Indian philosophical thought; they were not merely expositors... In this contribution, the author investigates the often heard claim that John Henry Newman influenced the teachings of Vatican II. The focus lies on the conciliar opening toward ecumenical dialogue, and the way in which Newman’s ideas have been received by an intermediary generation of theologians, who in turn brought them to the conciliar hall. In particular, this article illustrates how Newman’s legacy was received in French-speaking European milieus of the first half of the twentieth century. In order to do (...) so, the author studies the way in which Louis Bouyer, Yves Congar and Johannes Willebrands shared a common interest in Newman’s theological heritage, and how the presence of all three of these theologians in the Catholic Conference on Ecumenical Questions served as a gateway to Vatican II’s ecumenical turn. In an evolving global market of procreation, surrogacy is increasingly practiced and raises many ethical issues in terms of respect of the dignity of women, their bodily integrity and the consideration of the best interests and kinship of the unborn child. Faced with such ethical questions, the legal response in international law was tepid, while that of the States was fundamentally disparate, varying between systems of prohibition or legal framing of this practice and others that tolerate surrogacy but do not (...) regulate it. This article seeks to analyze the different ethical issues raised by surrogacy, then focuses on the various legal trends prohibiting or totally or partially authorizing this practice, and studying the legal treatment reserved by international law to this question. The 2008 financial crisis exposed the dark side of the financial sector in the UK. It brought attention to the contaminated culture of the business, which accommodated the systemic malpractices that largely contributed to the financial turmoil of 2008. In the wake of the crisis there seems to be a wide consensus that this contaminated culture can no longer be accepted and needs to change. This article examines the ills of the UK financial market, more specifically the cultural contamination problem, (...) which was uncovered by the 2008 financial crisis, in order to explore its genesis and the suitable solutions for it. In this regard, the article analyses the ethical finance sector from theoretical and practical perspectives in order to assess its role in addressing the cultural contamination problem of the UK financial market. This paper aims to measure the quality of all Romanian cities’ websites in 2019 and 2022, before and after the disruptive event of COVID-19. Since the official websites are the core instrument of e-governance, the changes in the quality of Romanian cities’ websites reflect the changes in the development of urban e-governance in Romania. The COVID-19 lockdowns and contact restrictions and the moving of most activities into the online environment had the potential to impact the performance of Romanian cities’ websites (...) significantly and catalyze the progress of local e-governance. The quality of Romanian cities’ official websites was measured using an adapted survey instrument proposed by Holzer and Kim and Manoharan, Melitski, and Holzer. The tool covers five critical criteria of website quality or performance: personal data security, usability, content, services, and citizen participation. The research results may seem contrary to expectations: the improvements of websites have been incremental rather than transformational. Although our research provides only a case study, we may assert that the reform of traditional administration remains a step-by-step process. Our study’s main contribution consists in showing that the progress of e-governance is an incremental process even in a situation of emergency that requires the moving of most activities into the online environment. The purpose of the present study was to compare differential functioning of Iranian English versus Persian teachers in responding to compliments and to investigate the possibility of sociolinguistic transmission of speech act of responding to compliments from English culture to native Iranian Persian speakers. Following Chen and Yang (2010), we hypothesized that exposure to English would affect the complimenting behavior of Persian speakers, leading to more acceptance of compliments compared to those with little or no exposure to English. Participants of (...) the study were 50 English teachers, regarded as “exposed-to-English” or E group, and 50 Persian teachers, considered as “unexposed-to-English” or U group. The participants’ age ranged from 20 to 35 and they were selected through non-randomized convenience sampling. A Discourse Completion Test (DCT) comprising eight complimenting situations was developed in two versions, i.e. English and Persian, in light of Mane’s (1983) taxonomy. The results revealed a significant difference in performance between groups and that Iranian English teachers’ Compliment Response (CR) behavior changed due to exposure to English and approached English culture. This paper attempts to analyze the performance of 57 memberstates of the Organization of Islamic Cooperation based on selectedindicators of some sectors namely demography, economics, educationand technology and innovation. Specifically, it aims at firstly portraying anoverview of OIC performance based on six selected indicators followed byanalyzing the relationship between selected development variables withliteracy and exploring the state of OIC performance as indicated by theirachievement based on selected indicators. The study was undertaken vis-àvisthe prevailing theories on modernization and development as well (...) as thewidely asserted underdevelopment of the contemporary Muslim ummah asclaimed by numerous contemporary Muslim scholars. The data were solicitedamong others from the World Bank Database, Statistical, Economic, and SocialResearch and Training, Centre of Islamic Countries of OIC andsome previous studies. They were then analyzed using SPSS with the resultsbeing generated mainly through the use of descriptive statistics. Among others,the study found that there is a positive correlation between Muslim countries’urban population growth and literacy rate, there is a steady decline in thenumber of OIC countries as they are categorized from lower to higher incomecategory placement and the percentage of the scientific publications of all 57OIC countries is far below that of any one single developed nation. IntroductionIn the past three decades, there has been increasing interest in assessing children’s Executive Functions. However, studies on the conceptualization and operationalization of this construct are incongruent and guidance for clinicians and researchers aiming to assess EF is insufficient due to measurement variability.AimsThe purpose of this article was to examine current theories and models of EF in children, identify their assessment instruments, issues, and challenges, and discuss their impact on children’s cognitive, behavioral, social and/or emotional development.MethodsThis narrative review reflected on (...) English and French scholarly articles on EF assessment in children. References were identified through searches of PubMed, Medline, ScienceDirect, Google Scholar, and APA PsychNet throughout the last two decades up to June 2022.ResultsThere are commonalities despite divergence in the definition and operationalization of EF. Assessment of EF requires psychometric tests as well as rating scales that must be integrated and interpreted considering the child’s biological makeup, environmental background, and cultural specificities.ConclusionCurrent EF theories, assessment tools, issues, and challenges were discussed in addition to the impact of their components’ dysfunctions on children’s development. Further studies should be conducted to develop new measurement methods and technologies to improve the ecological and ethological validity of youth assessment, treatment, and interventions. (shrink)

Das Anliegen der vorliegenden Studie ist der Entwurf eines wirklichkeitstreuen Modells der Kommunikation. Ebenfalls hat uns interessiert, die konstitutiven Regeln der Kommunikation zu bestimmen und einige Wirksamkeitsnormen und moralische Normen, die mit wünschenswerten sozialen Kommunikationsformen assoziierbar sein könnten, zu identifizieren. Die Kommunikation ist ein facettenreiches und zugleich interpretationsoffenes Phänomen, welches zahlreiche unterschiedliche theoretische Modelle erlaubt. Sie kann von den Psychologen als selbstständigen Verhaltenstyp, von den Soziologen als entscheidenden Sozialisierungsfaktor, von den Anthropologen als kulturhervorbringendes und -verbreitendes Mittel, von den Semiologen als (...) Prozess der (Um-)Konfiguration der verschiedenen Bedeutungen von Bedeutungsträgern usw. gedeutet werden. Das vorliegende Interpretationsmodell der Kommunikation ist allgemein, zum Teil auch hermeneutisch, indem es a) den Kommunikationsprozess als solchen und nicht etwa als eine gesonderte Instanz desselben berücksichtigt, b) eine einzige, nämlich die interaktive Perspektive auf die Kommunikation hervorhebt und c) zweckgemäß die Kommunikation in einen normativen, intelligiblen Rahmen überträgt, wobei sich eine empirische Beschreibung der Kommunikationsfakten zwecks Feststellung allgemeingültiger Gesetze von selbst erübrigt. Diesem theoretischen Modell entsprechend ist die Kommunikation eine Interaktion oder eine wechselseitige semiotische Interaktion, innerhalb derer die involvierten Personen mithilfe von Zeichen komplementäre Verhalten manifestieren, die bestimmten (sozialen) Regeln unterworfen sind. Demzufolge ist die Kommunikation kein individuelles Spiel, sondern ein Team-Spiel. Ihre Protagonisten spielen alle genauso wichtige Rollen, und von ihrer Teilnahme hängt beides, Gelingen und Misslingen der Kommunikation, gleichermaßen ab. Nachdem wir den Begriff der Kommunikation im allgemeinen erklärt hatten, galt unser Interesse der Definition und der Klassifizierung der exklusiven Ausführungsmittel des oben beschriebenen Prozesses, das heißt, der Zeichen. Die semiotische Literatur behandelt die Zeichen sowohl als (stoffliche, mentale oder gemischte) Objekte als auch als Funktionen. Für unsere Demarche haben wir es vorgezogen, die Zeichen als in Wirklichkeit existierende Reize zu beschreiben, die auf Grund von sozialen Konventionen stets auf „etwas Anderes” als auf sich selbst hinweisen. Dieses gemeinte „Andere“, die Bedeutung des Zeichens, ist nichts anderes als die Gesamtheit seiner Anwendungen. Die Natur des Verhältnisses von Zeichen und (willkürlicher, notwendiger, durch Analogie oder Ähnlichkeit begründeter) Bedeutung hat uns dann erlaubt, vier Zeichenkategorien auszumachen und zu beschreiben: Wörter, Indizien, Bilder und Sinnbilder. Was die Erläuterung der konstitutiven Faktoren der Kommunikation anbetrifft, haben wir im Einklang mit den Forschungen angesehener Semiotiker mit sechs grundlegenden Parametern gerechnet: Sender, Empfänger, Mitteilung, Code, Kommunikationssituation und Kommunikationskanal. Doch war das wirklich die einzige treffliche Option? Hätte der Kommunikationsprozess nicht ausgehend von anderen – wenigeren oder mehreren – Faktoren analysiert werden können? Gewiss kann die Kommunikation auch mittels weiterer Variablen verstanden werden – und die Fachliteratur ist nicht müde geworden, die Vorzüge anderer Beschreibungsmuster zu zeigen –; von den sechs semiotischen, von Roman Jakobson eingeführten Parametern kann man indes nicht wegsehen. Sie erlauben allemal eine ziemlich genaue Bestimmung der durch die Protragonisten der Kommunikation durchgeführten Handlungen. In Übereinstimmung mit der allgemeinen Methode der Handlungstheorie haben wir jeden konstitutiven Faktor der Kommunikation mit je einem Handlungspaar korreliert, zum einen beim Sender, zum anderen beim Empfänger. Die Berücksichtigung der Sprache und implizite der Kommunikation im Kontext der Handlungstheorie ist keine bahnbrechende Unternehmung, da sie unter anderem schon John Austin, John Searle und Jürgen Habermas als Ausgangspunkt ihrer Analyse der Sprechakte gedient hat. Auch die Korrelation der Kommunikationsparameter mit einer Handlung ist kein Novum, indem sie auf analoge Weise bereits durch die Versuche einiger Semiotiker wie Roman Jakobson und Karl Bühler, die Funktionen der Sprache bzw. des linguistischen Zeichens zu systematisieren, unternommen wurde. Unser origineller Beitrag besteht aus der Korrelation jedes Faktors der Kommunikation mit einer Interaktion, das heißt mit einem Paar von komplementären Handlungen, so dass jeder Handlung des Senders eine Handlung des Empfängers entspricht. In diesem Sinne haben wir Sender, Empfänger, Mitteilung, Code, Kommunikationssituation und Kommunikationskanal in spezifischen Interaktionstypen (Sichentlarven – Nachvollziehen, Einflussnahme – Antwort, Sendung – Empfang, syntaktische Informierung – syntaktisches Verständnis, Bezeichnung – „Mitbezeich-nung“ bzw. Anknüpfung einer soziale Beziehung – Beibehaltung einer soziale Beziehung) inszeniert. Nachdem wir den Kommunikationsprozess als Totalität von sechs Interaktionstypen beschrieben hatten, haben wir auf drei mögliche Ordnungen der Kommunikationsformen verwiesen, wobei die Einteilungskriterien jeweils die verschiedenen gebrauchten Zeichentypen, die Art der Beziehung zwischen Sender und Empfänger bzw. der sich spontan manifestierende soziale Abstand zwischen den Kommunizierenden waren. Indem unsere Demarche darauf abzielte, die Eigentümlichkeiten verbaler vs. nonverbaler, formaler vs. informaler, der individuell, gruppenweise oder öffentlich stattfindenden Kommunikation auszumachen, haben wir die Akzente auf die Pragmatik der Kommunikation gesetzt und somit einige Hinweise auf den Erwerb spezifischer Kommunikationsfähigkeiten geben können. Das von uns vorgeschlagene theoretische Modell hat also nicht nur ein besseres Verständnis der Kommunikation beabsichtigt, sondern auch die Feststellung eines vollständigen Regelwerks der Kommunikation. Diese Regeln wurden als Formen des zu erwartenden sozialen Verhaltens behandelt – sowohl im normativen als auch im antizipativen Sinne – und wurden im Gefolge von John Searle in zwei Hauptklassen eingeteilt: konstitutive und normative Regeln. Die konstitutiven Regeln bestimmen die von ihnen logisch-abhängigen Verhaltensweisen und legen die Bedingungen der Kommunikationsakte fest, während die normativen Regeln die vorgegebenen, unabhängigen Verhaltensweisen berücksichtigen und die wünschenswerten Bedingungen für ihre Vollziehung festlegen. Um die konstitutiven Regeln ausmachen zu können, sind wir davon ausgegangen, dass der Prozess der menschlichen Kommunikation kein naturhafter, sondern ein institutioneller ist. Mit anderen Worten ist die Kommunikation das Ergebnis einer sozialen Vereinbarung und eben kein unabhängiges Phänomen, das nur von außen zu beschreiben wäre. Je nachdem, wie die notwendigen Regeln, welche die soziale Vereinbarung definieren, respektiert oder verletzt werden, haben wir fünf mögliche Situationen identifiziert: die vollkommene Kommunikation, die teilweise geglückte Kommunikation, die Nonkommunikation, die relative und die absolute Antikommunikation, wobei man hinzufügen muss, dass die erste und die letzte Situation Grenzerfahrungen ohne jegliche Referenz darstellen. Die konstitutiven Regeln der Kommunikation wurden mit allen sechs spezifischen Interaktionstypen korreliert (Sichentlarven – Nachvollziehen, Einflussnahme – Antwort, Sendung – Empfang, syntaktische Informierung – syntaktisches Verständnis, Bezeich-nung – „Mitbezeichnung“ bzw. Anknüpfung einer soziale Beziehung – Beibehaltung einer soziale Beziehung), so dass sie wenigstens aus einem bestimmten Gesichtspunkt ein konsistentes und vollständiges System bilden können. Im Unterschied zum System der konstitutiven Regeln sind hier die Kommunikationsnormen nicht erschöpfend behandelt worden. So wurden aus der Vielfalt der Vorschriften, die zu erwünschten Ergebnissen in der Kommunikation führen könnten, nur einige im Rahmen der vorliegenden Studie angegangen. Diese betreffen die Wirksamkeit und die moralischen Aspekte der Kommunikation und wurden ihrerseits mit den oben angeführten sechs Interaktionstypen korreliert. Dabei waren nicht so sehr die unausweichlichen Lücken im Inventarisieren der Normen, welche die Genauigkeit und die Ökonomie der Kommunikation gewähren, am problematischsten, sondern vielmehr der Grad an moralischer Relevanz, den die Kommunikationsakte beanspruchen können. Wir sind fest davon überzeugt, dass die Handlungen der Kommunizierenden eher davon abhängen, inwiefern sie moralische Verpflichtungen eingehen, und eben nicht davon, was für einen Gewinn sie dadurch erzielen, da keine der berücksichtigten Handlungen sich der Moral gänzlich entziehen können. Ob „neutrale Metaregeln“ für den Fall, dass einige moralische Eingrenzungen mit den Wirksamkeitsnormen kollidieren, vonnöten sind, sei dahin gestellt. Ein theoretisches Modell kann nur dann als nützlich bezeichnet werden, wenn es neue Forschungsansätze bringt und neue Forschungswege ermöglicht. Unter diesem Blickwinkel hat die vorliegende Studie zwei mögliche Fortsetzungen: die Ergänzung der Gesamtheit der normativen Regeln der Kommunikation (im allgemeinen), bzw. das Unterteilen des allgemeinen Modells der Kommunikation in stereotypen Mustern wie der politischen oder didaktischen Kommunikation usw. The work presented here investigates the process by which one group of individuals solves the problem of detecting deceptions created by other agents. A field experiment was conducted in which twenty-four auditors (partners in international public accounting firms) were asked to review four cases describing real companies that, unknown to the auditors, had perpetrated financial frauds. While many of the auditors failed to detect the manipulations in the cases, a small number of auditors were consistently successful. Since the detection of (...) frauds occurs infrequently in the work of a given auditor, we explain success by the application of powerful heuristics gained from experience with deceptions in everyday life. These heuristics implement a variation of Dennett's intentional stance strategy, which is based on interpreting detected inconsistencies in the light of the Deceiver's (i.e., management's) goals and possible actions. We explain failure to detect deception by means of perturbations (bugs) in the domain knowledge of accounting needed to apply these heuristics to the specific context of financial statement fraud. We test our theory by showing that a computational model of fraud detection that employs the proposed heuristics successfully detects frauds in the cases given to the auditors. We then modify the model by introducing perturbations based on the errors made by each of the auditors in the four cases. The resulting models account for 84 of the 96 observations (i.e., 24 auditors × four cases) in our data. (shrink)

Viewing workplace deviance within a victim precipitation framework, we explore how abusive supervisors target subordinates low in core self-evaluations to explain when such employees respond by engaging in workplace deviance. We theorize that employees who are lower in CSE receive more abusive supervision, which generates subsequent harmful reactions toward supervisors, peers, and the organization. This occurs primarily when employees lack sufficient cognitive resources in dealing with supervisor abuse. We test, replicate, and extend our theoretical model in three empirical studies. Results (...) demonstrate that lower employee CSE drew more abusive supervision and led low-CSE employees to exhibit workplace deviance. This abusive supervision mediation effect was stronger for employees with comparatively lower cognitive ability levels. In order to promote better practices and communication around end-of-life decision-making, several Canadian hospitals in the province of Quebec have developed a tool called “Levels of Intervention”. No work to date has been published demonstrating improvement since these forms were implemented. The purpose of the present study was to obtain information about the use of LOI forms across Quebec hospitals and to identify gaps in practice as well as areas for improvement. A retrospective study was undertaken of 299 charts of (...) patients who had died in three Quebec hospitals with a LOI ordered. Results were analysed through a principlism ethical framework. High compliance with the level of intervention ordered at the time of death was recorded, as well as high involvement of patient and/or family, demonstrating the efficiency of LOI in promoting respect for autonomy. Other results show delays in end-of-life care discussions in the course of the hospitalization. Only a small proportion of patients who died had a palliative care consultation, which may reflect equity issues in access to care. This study highlights the importance of the LOI in Quebec and the role it is playing in respect for end-of-life preferences as well as in the involvement of patients and families in the decision-making process. Training specific to end-of-life decision-making conversations would help support the LOI form’s use, as would developing provincial or national guidelines on the use of LOI to standardize organizational policies and practice around end-of-life care. Background: SNOMED CT is a large terminology system designed to represent all aspects of healthcare. Its current form and content result from decades of bottom-up evolution. Due to SNOMED CT’s formal descriptions, it can be considered an ontology. The Basic Formal Ontology (BFO) is a foundational ontology that proposes a small set of disjoint, hierarchically ordered classes, supported by relations and axioms. In contrast, as a typical top-down endeavor, BFO was designed as a foundational framework for domain ontologies in the (...) natural sciences and related disciplines. Whereas it is mostly assumed that domain ontologies should be created as extensions of foundational ontologies, a post-hoc harmonization of consolidated domain ontologies in use, such as SNOMED CT, is known to be challenging. Methods: We explored the feasibility of harmonizing SNOMED CT with BFO, with a focus on the SNOMED CT Clinical Finding hierarchy. With more than 100,000 classes, it accounts for about one third of SNOMED CT’s content. In particular, we represented typical SNOMED CT finding/disorder concepts using description logics under BFO. Three representational patterns were created and the logical entailments analyzed. Results: Under a first scrutiny, the clinical intuition that diseases, disorders, signs and symptoms form a homogeneous ontological upper-level class appeared incompatible with BFO’s upper-level distinction into continuants and occurrents. The Clinical finding class seemed to be an umbrella for all kinds of entities of clinical interest, such as material entities, processes, states, dispositions, and qualities. This suggests the conclusion that Clinical finding would not be a suitable upper-level class from an BFO perspective. On closer inspection of the taxonomic links within this hierarchy and the implicit meaning derived thereof, it became clear that Clinical finding classes do not characterize the entity (e.g. a fracture, allergy, tumor, pain, hemorrhage, seizure, fever) in a literal sense but rather the condition of a patient having that fracture, allergy, pain etc. This gives sense to the current characteristic of the Clinical Finding hierarchy, in which complex classes are modeled as subclasses of their constituents. Most of these taxonomic links are inferred, as the consequence of the ‘role group’ design pattern, which is ubiquitous in SNOMED CT and has often been subject of controversy regarding its semantics. Conclusion: Our analyses resulted in the proposal of (i) equating SNOMED CT’s ‘role group’ property with the reflexive and transitive BFO relation ‘has occurrent part’; and (ii) reinterpreting Clinical Findings as Clinical Occurrents, i.e. temporally extended entities in an organism, having one or more occurrents as temporal parts that occur in continuants. This re-interpretation was corroborated by a manual analysis of classes under Clinical Finding, as well as the identification of similar modeling patterns in other ontologies. As a result, SNOMED CT does not require any content redesign to establish compatibility with BFO, apart from this re-interpretation, and a suggested re-labeling. Regarding the feasibility of harmonizing terminologies with principled foundational ontologies post-hoc, our results provide support to the assumption that this does not necessarily require major redesign efforts, but rather a careful analysis of the implicit assumptions of terminology curators and users. This study aims at assessing the level of knowledge, attitudes, and practices (KAP) concerning reproductive health (RH) among adolescent college-going girls in the urban and rural areas of Chittagong District, Bangladesh. A college-based cross-sectional study was conducted among college-going girls (N = 792) of four colleges attending Higher Secondary Certificate (HSC) classes (eleven/twelve classes) in Chittagong District. Data were collected using a structured and self-administered questionnaire. Chi-square (χ2) and independent-samples t-test were conducted to make the comparison between urban and rural (...) participants. Data were analyzed using IBM SPSS version 24.0. The authors’ interpretation of the findings was also informed by anthropological as well as qualitative considerations. Mothers were reported to be the key informants of reproductive health for the adolescent girls of both urban and rural areas. Besides, urban respondents were more likely than rural respondents (p <.001) to feel comfortable during the discussion on RH with mother/sister/relative; to communicate frequently with their mothers; to discuss RH issues regularly; to have prior knowledge on menstruation. Moreover, this study also found significant differences (p <.05) regarding most of the items addressing RH knowledge, attitude, and practice between adolescent girls in urban and rural areas of Bangladesh due to the existing socio-cultural disparities between the areas of residence. Background: Despite an increase in health research conducted in Africa, there are still inadequate human resources with research ethics training and lack of local long-term training opportunities in research ethics. A research ethics training program named United States-Mali Research Ethics Training Program (US-Mali RETP) was established through a partnership between the George Washington University Milken Institute School of Public Health (GWSPH), USA and University of Sciences, Techniques & Technologies of Bamako (USTTB), to address the critical need for improved bioethics training, (...) leadership, and policy in Mali. Methods: The aims of the capacity building programme are achieved by leveraging US and Africa-based expertise to strengthen research ethics education and capacity through: (1) intensive faculty development to design ethics training curricula; (2) development of a research ethics specialization within the existing Masters of Public Health program (MPH); and (3) establishing professional development courses through short-term training workshops and webinars to address research and professional needs in ethics. Results: The program will strengthen USTTB’s capacity of research ethics by training at least 15 MPH students over 5 years to prepare the next generation of ethics researchers. During the first two years of the program, a new ethics training curricula was developed, two cohorts of Master’s students were enrolled, and a series of webinars and workshops were conducted with participation of Malian and international researchers. Conclusions: US-Mali RETP will promote a sustainable bioethics enterprise at USTTB, and enable dissemination of research ethics training to increase health research capacity in Mali. (shrink)

Childhood adversity is associated with altered or dysregulated stress reactivity; these altered patterns of physiological functioning persist into adulthood. Evidence from both preclinical animal models and human neuroimaging studies indicates that early life experience differentially influences stressor-evoked activity within central visceral neural circuits proximally involved in the control of stress responses, including the subgenual anterior cingulate cortex (sgACC), paraventricular nucleus of the hypothalamus (PVN), bed nucleus of the stria terminalis (BNST) and amygdala. However, the relationship between childhood adversity and the (...) resting-state connectivity of this central visceral network remains unclear. To this end, we examined relationships between childhood threat and childhood socioeconomic deprivation, the resting-state connectivity between our regions of interest (ROIs), and affective symptom severity and diagnoses. We recruited a transdiagnostic sample of young adult males and females (n= 100; mean age = 27.28,SD= 3.99; 59 females) with a full distribution of maltreatment history and symptom severity across multiple affective disorders. Resting-state data were acquired using a 7.2-min functional magnetic resonance imaging (fMRI) sequence; noted ROIs were applied as masks to determine ROI-to-ROI connectivity. Threat was determined by measures of childhood traumatic events and abuse. Socioeconomic deprivation (SED) was determined by a measure of childhood socioeconomic status (parental education level). Covarying for age, race and sex, greater childhood threat was significantly associated with lower BNST-PVN, amygdala-sgACC and PVN-sgACC connectivity. No significant relationships were found between SED and resting-state connectivity. BNST-PVN connectivity was associated with the number of lifetime affective diagnoses. Exposure to threat during early development may entrain altered patterns of resting-state connectivity between these stress-related ROIs in ways that contribute to dysregulated neural and physiological responses to stress and subsequent affective psychopathology. The objective of the current study was to propose a sensitivity analysis of a 3D left ventricle model in order to assess the influence of parameters on myocardial mechanical dispersion. A finite element model of LV electro-mechanical activity was proposed and a screening method was used to evaluate the sensitivity of model parameters on the standard deviation of time to peak strain. Results highlight the importance of propagation parameters associated with septal and lateral segments activation. Simulated curves were compared to (...) myocardial strains, obtained from echocardiography of one healthy subject and one patient diagnosed with intraventricular dyssynchrony and coronary artery disease. Results show a close match between simulation and clinical strains and illustrate the model ability to reproduce myocardial strains in the context of intraventricular dyssynchrony. (shrink)

In this systematic review, we collected and analyzed literature works comparing self-reported fatigue and objectively-measured fatigue in individuals with cerebral palsy and in age-matched typically developing/typically developed controls. The search was conducted on four electronic databases/platforms using the key words “cerebral palsy” combined with “fatig*,” where the asterisk was used as a wildcard. As a critical appraisal tool, the Joanna Briggs Institute Critical Appraisal Checklist for Quasi-Experimental Studies was used. A total of 22 studies passed the critical appraisal rating and (...) were included in both narrative and quantitative analyses. The overall evidence quality of the findings was considered very good. Data of objectively-measured fatigue in performing maximal fatiguing tasks indicated lower fatigue levels in participants with CP, possibly due to their pathological inability to recruit highly fatigable muscle fibers. Highly trained individuals with CP and TD controls performing maximal fatiguing tasks seem to be an exception to this, as they exhibited similar levels of fatigue. In submaximal fatiguing tasks, including daily physical activities, either objectively-measured or self-reported fatigue was higher in participants with CP than in TD controls, indicating a lower ability for development of neurophysiological compensation for fatigue among participants with CP. Further studies on fatigue are needed to gain an insight into the multifold mechanisms of fatigue in individuals with CP. Understanding fatigue mechanisms could help in setting up strategies for effective intervention programs, with benefits in healthcare and improved quality of life of individuals with CP.Systematic Review Registration: [PROSPERO 2019], identifier [CRD42019143524]. Muhammad al-Shahrastani, the famous Muslim theologian of the 12th century and author of the Book of Religious and Philosophical Sects, was greatly influenced by Ismaili teachings. In this work al-Shahrastani refutes the metaphysics of Ibn Sina (Avicenna) from an Ismaili point of view.

This study aimed to examine the relationship between anxiety, depression, subjective well-being, and academic performance in Peruvian university health science students with COVID-19-infected relatives. Eight hundred two university students aged 17–54 years ; 658 females and 144 males ; who completed the Patient Health Questionnaire-2, Coronavirus Anxiety Scale, Subjective Well-being Scale, and Self-reporting of Academic Performance participated. A partial unregularized network was estimated using the ggmModSelect function. Expected influence values were calculated to identify the central nodes and a two-tailed permutation (...) test for the difference between the two groups. The results reveal that a depression and well-being node presents the highest relationship. The most central nodes belonged to COVID-19 anxiety, and there are no global differences between the comparison networks; but at the local level, there are connections in the network of COVID-19-infected students that are not in the group that did not present this diagnosis. It is concluded that anxious–depressive symptomatology and its relationship with well-being and evaluation of academic performance should be considered in order to understand the impact that COVID-19 had on health sciences students. This paper attempts to rethink the philosophy of Muhammad Iqbal (1877–1938) and challenge the still prevailing tendency in Iqbal scholarship to view it merely as an outcome of the influence of the ideas of various Western/European philosophers. I present Iqbal’s arguments in their particular historical and intellectual context to show that they developed in response to a specific philosophical problem and that Iqbal looked for a solution to that problem in Islamic tradition. I suggest that Iqbal’s philosophy is best understood (...) in the context of, and as a response to, the problem of nihilism as it was debated in modern German philosophy during ‘the pantheism controversy’ in the eighteenth and nineteenth centuries. To demonstrate this, I analyse Iqbal’s article on ʿAbd al-Karīm al-Jīlī to show his concern with the problem of nihilism, and his solution to it based on al-Jīlī’s Sufism. (shrink)

The Commentary on Sefer Yeṣira, with its pronounced Pythagorean and Neo-Platonic overtones, written by Saadya Gaon in 931, stands out among the other writings of this Jewish theologian, and raises the question of the purpose of its composition. It has been argued that in writing a commentary on this work of letter-speculation, Saadya responded to mythical and mystical trends in tenth-century Judaism, endeavoring to recast this foundational mystical text as a work of rational philosophy. The present article argues that Saadya (...) was also responding to the intellectual challenge of his broader environment, stretching beyond the Jewish community. In some circles in the Islamicate world, letterspeculations, often associated with the sciences of the occult, were presented in this period as the height of philosophy. In particular, al-Tawḥīdī’s account of the Pure Brethren and Ibn Masarra’s Book on the Properties of Letters demonstrate the relevance of these trends in Saadya’s immediate geographic and intellectual environment. This article proposes an alternative approach in sustainability assessment. The conceptual framework was developed by modifying Ken Wilber's All Quadrants, All Levels (AQAL) approach, and focuses on the inter-relatedness/inter-connection of various perspectives inherent to the concept of sustainability. To look at how our framework can facilitate the practice of sustainability assessment, we apply the framework to examine the relationships between social systems and the environmental changes in the Ili River basin across the period 1900?2008. This approach enables us to investigate (...) the environmental problems of the Ili River basin in a four-quadrant framework, and combine the empirics of quadrants obtained from traditional disciplinary methodologies. The four-quadrant framework adopted in this study illustrates the interlocking relationships among various perspectives of environmental issues in the Ili River basin, namely, physical perspective, personal perspective, cultural perspective, and social perspective. In particular, the protruding development (evolution) of the lower right dimension is the fundamental cause of the environmental degradation and its related social problems in the Ili River basin. Compared to other established approaches in literature that emphasize on the tradeoffs of various perspectives of sustainability, our findings indicate the potential contributions of four-quadrant framework to sustainability assessment through its focus on the inter-relatedness/inter-connection of different perspectives. İsrā’iliyyat which is the narration material passed on to Islam from other religions and cultures, primarily Judaism and Christianity has been one of the topics discussed especially within the frame of tafsir discipline throughout the fourteen centuries of Islamic history. While these narrations were used as one of the sources of information without being subjected to a normative evaluation in the early day tafsirs, they have been started to be criticized emphasizing mainly on the weakness of their imputation in the (...) classical tafsirs. When it was reached to the modern era, with the effect of movements of thoughts such as Western originated positivism, rationalism, and historical criticism, and orientalist allegations. İsrā’iliyyat has been criticized rather content-wise, reviewed as unreal, imaginary, mythological story-type narrations mixed with superstitions. As a result of these criticisms, both a distance has been kept from İsrā’iliyyat and also alternatives to it have been pursued. Although in the modern-day tafsirs, it has been consulted to the Bible as the strongest alternative to İsrā’iliyyat, this situation has made way for many debates, and it has dwelled on other alternatives than that. These points mentioned are valid for Tunisian scholar Tāhir İbn ʿĀshūr’s (1879-1973) tafsir named al-Tahrīr wa’t-Tanwīr, too. An important feature of this tafsir is that although it has been looked kindly upon İsrā’ili narrations within the frame of certain criteria, it has been criticized in general. The writer has developed alternatives to İsrā’ili narrations without complimenting them as much as possible. Among these alternatives, while he prioritized the Bible narrations, he didn’t settle for this and he headed for other alternatives, too. In this study, it has been emphasized on methods of text-centered commentary, socio-psychologic commentary, and foresight-based commentary, which İbn ʿĀshūr used as an alternative to İsrā’iliyyat from time to time. It has been seen that the writer developed stronger and farther-from-a-debate-ground methods compared to İsrā’iliyyat and used these methods successfully in this way. It has come to the conclusion that in the case of these methods being used in Qura’ân parables accurately, they can be an important alternative to İsrā’iliyyat. (shrink)

Polygamy, which was practiced without limitations in the past, had been restricted to four wives after the arrival of Islam in the Arabian Peninsula. However, some scholars have different views on this issue, supposedly influenced by the literal and cultural background of patriarchal tradition on treating women as the object of polygamy. This article attempts to examine the construction of patriarchal interpretation in a gender-biased interpretation, its factors and its implications. This study adopts a qualitative approach and employs a content (...) analysis approach. Interviews with relevant scholars are also used to explore in-depth information regarding this subject. The object of research is based on a full set (30 juz /chapters) of 11 books of tafsir written in Indonesia. These books are Tafsir Al-Furqan, Tafsir Quran, Adz-Dzikra: Terjemah dan Tafsir al-Quran, Al-Quran dan Terjemahnya, Tafsir Quran Karim, Tafsir al-Azhar, Tafsir Rahmat, Tafsir al-Quran al-Majid an-Nur, Tafsir al-Mishbah, Al-Quran and its Exegesis of the Department of Religion, and Tafsir al-Hijri. This study found that women have been exploited through polygamy practices. Our findings show that biased gender interpretation, especially because of the patriarchal mindset, brought a greater impact on the Quranic interpretation. This study suggested that reinterpretation towards Quranic verses particularly dealing with gender issues needs to be strengthened in accordance with justice and humanistic values. Contribution: This article offered two approaches for the study of the Quran in order to establish gender equality and justice in marriage practice. Firstly, the adaptive and humanistic interpretations of the Quran need to be strengthened and should raise common consciousness in Muslim society. Secondly, there is a need to study the Quran in an integrative, holistic and hermeneutical understanding of the Quranic text. This method can explore the deepest meaning of the Quran so it can rise gender-sensitive, humanist and moderate interpretations. (shrink)
